NEW CLENTS


We welcome new patients!

At Trooper Veterinary Hospital, we try to see you at your appointed time as much as possible because we believe your time is as valuable as ours.  The following information can help us accomplish that goal.

We are located in a heavy traffic area.  Please allow for traffic congestion and plan ahead to arrive at least 15 minutes early for your first visit. 

In order to save time, if possible, please print out the following information to help us prepare for your first visit.

Please download and complete our NEW CLIENT INFORMATION FORM and , if possible fax it to us at 610-539-4851 or drop it off so we have it the day before your appointment.  If you bring the form at your visit please be sure to arrive 15 minutes early. 

If you have you have any problems printing this form, we can fax a copy to you, or mail if time allows.  If you want to fill out your paperwork upon arrival, we ask that you arrive AT LEAST 20 minutes prior to your appointment

 

Bring any previous pet records for us to copy, or have them faxed PRIOR TO YOUR APPOINTMENT by your previous vet.  610-539-4851.  Having your records ahead of time for the doctor to review will help to prevent delay in your appointment.  

Please bring a fresh (not more than 12 hours old) stool sample if you are bringing a new puppy or kitten, or if your pet is experiencing intestinal problems.

Complete one New Client Form form for each pet.  If you have multiple pets, the owner information can be completed on just one of the pet forms.

Visit our directions page for our location and please call us anytime if we can be of assistance.  Our staff is happy to help you with any questions you may have.  

Please check in at the desk upon arrival and enjoy our free coffees and teas as well as separate dog and cat waiting areas.
