We welcome new patients!

At Trooper Veterinary Hospital, we try to see you at your appointed time as much as possible because we believe your time is as valuable as ours.  The following information can help us accomplish that goal.

Please download and complete our NEW PATIENT INFORMATION FORM and if possible drop it off or fax/e-mail it to us along with you pet's medical records.  Having your records ahead of time for the doctor to review will help to prevent a delay in your appointment.  

If you have any problems printing this form, we can fax or e-mail it to you, or mail if time allows.  If you want to fill out your paperwork upon arrival, we ask that you arrive AT LEAST 20 minutes prior to your appointment

Registration takes about 15 minutes after we have all of your forms and must be completed prior to your doctor appointment time. 

We are located in a heavy traffic area.  Please allow for traffic congestion and plan ahead to arrive at least 15 minutes earlyfor your first visit.  In fairness to other clients, if you arrive late, we will try to accommodate you by seeing you in the next available appointment space, but keep in mind this can result in a considerable wait.  Please call us ahead of time if you need assistance with directions.

Please bring a fresh (not more than 12 hours old) stool sample if you are bringing a new puppy or kitten, or if your pet is experiencing intestinal problems.

Complete one New Patient Form form for each pet.
